• A Biographical Sketch
• A statement of personal and professional goals
• Essential Functions Form
• Official transcripts from all colleges attended
• TOEFL or IELTS is required for students whose native language is not English.  (This requirement is waived for students who have successfully completed 12 credit hours or more of coursework in a regionally accredited institution of higher education.)
