Transcripts

Due to the fact that a signature is required when making this request, an online request form is not available. However we have made a printable copy of the official transcript request form available on this page. This form is PDF format. If you need the Adobe Acrobat Reader, you may download it for free here.

Transcripts sent directly to the student are stamped "issued to student," and may not be considered an official transcript by some institutions.  Students should check with the institution to verify their requirements for receipt of the transcript.
